In order to apply and be qualified for an award of the Bronze
Star Medal as a conversion award based on your Combat
Infantryman Badge or Combat Medic Badge you must have
your discharge papers showing the award of the CIB or
CMB or a copy of the General Orders making the award.
Requests for this award must be initiated either by the
individual or his primary next of kin (Spouse, Eldest Child,
Mother/Father) Oldest Sibling or Eldest Grandchild in that
order) If a relative you must state how you are related to the
individual to whom the award will made.
Send your request to the US Army Human Resources
Command, ATTN: AHRC-PDP-A, 200 Stovall Street,
Alexandria, VA 22332-0471. Expect several months for
your request to be processed in that the office responsible
for final processing at Department of the Army is also
responsible for processing awards for soldiers serving in Iraq.
Make a copy of your discharge or General Order and send it
along with a letter as follows:

Dear Sir:
Under the provisions of AR 600-8-22, paragraph 3-13d,
request is made for a conversion award of the Bronze Star
Medal (BSM) for (insert your name, rank and serial number)
(If not being submitted by the Service member- indicate here
what your relationship is to the deceased member)
Documentary evidence is enclosed to substantiate the award
of the (Combat Infantryman Badge or Combat Medical
Badge) on (Date) entitling me(or him) to the BSM as a
conversion award.
Please mail orders and award to the following address;(insert
your mailing address).
Thank you for your assistance in this matter.
